Purpose: The questionnaire is designed to address key areas of life during your tour. In recognition of the fact that no two tours are the same, you might like to fill out a new questionnaire for each new tour. Please feel free to copy this form to your word processor, complete the form and then EMail it to me at:
F13b@aol.com
The information you provide will be extracted and added to the site. Stories will be published on the Bulletin Board if you have not already published them there yourself (the detail in them will also form the meat of any analytical articles published on the website). By contributing, you are ensuring that our hard work (and hard play) do not go unremembered.
If you do not want your answers published, please say so on the bottom of the form.
